Bring these forms on your first day of class:

2. Road Test Record of Examination Form (pdf)

Note: Print, fill in all pdf files, date it, initials and sign all forms and place it in a folder 📂.

On the 1st day of your in car sessions, we will be requesting all the original  copies of all the above pdf files submitted by the student via email, so that the instructor can submit all the documentation to MTO government; in order for the student to attain their insurance certification benefits.
